- Information you provide about your emotional and mental wellbeing during intake and assessment.
- Information about your use of our evaluation services, including appointment and documentation status.
- Contact and account details linked to the above.
2. Why we collect it
Solely to provide the services you request: matching you with a licensed clinician, facilitating your evaluation, delivering documentation, processing payment and refunds, providing support, and meeting legal obligations.
3. Sources and sharing
We collect consumer health data directly from you. We share it only with the independent licensed clinician conducting your evaluation and with service providers bound by contractual confidentiality who help us operate (hosting, payments, communications). We do not sell consumer health data, and we do not share it for third-party advertising. Where a law requires consent for sharing, we obtain it.
4. Your rights
- Confirm whether we collect, share, or process your consumer health data.
- Access your data, including a list of third parties with whom it was shared.
- Withdraw consent and request deletion of your consumer health data.
- Appeal a decision by emailing privacy@mypetesa.com with "Appeal" in the subject line.
We respond to verified requests within the timeframe your state’s law requires and never discriminate against you for exercising your rights.
