This notice applies to health information created or received in connection with telehealth evaluations facilitated through MyPetESA by independent licensed clinicians ("providers"). Each provider is responsible for their own clinical records under HIPAA and their state’s laws; MyPetESA safeguards information it handles on their and your behalf.
2. How your health information may be used
- Treatment: your provider uses your information to conduct your evaluation and, where appropriate, issue documentation.
- Payment: to process your evaluation fee and any refunds.
- Health care operations: quality review, license verification, platform security, and support.
4. Disclosures permitted or required by law
Limited disclosures may occur when required by law: to avert a serious threat to health or safety, for oversight by licensing authorities, in response to valid legal process, or as otherwise mandated by state and federal law.
5. Your rights
- Request access to and a copy of your health information.
- Request corrections to information you believe is inaccurate.
- Request restrictions on certain uses and confidential communications.
- Receive an accounting of certain disclosures.
- Receive a copy of this notice and file a complaint without retaliation.
6. Complaints
If you believe your privacy rights were violated, contact us at privacy@mypetesa.com or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. You will never be penalized for filing a complaint.
